Direct Patient Care & Medical Management

  • Assessments: Conduct thorough physical, psychological, and cognitive assessments to identify patient needs.
  • Care Planning: Develop, implement, and regularly review personalized care plans based on assessment findings and physician orders.
  • Medication Administration: Administer all prescribed medications, monitor for side effects, and maintain accurate records.
  • Treatment & Wounds: Change dressings, treat wounds, and perform other treatments as directed.
  • Monitoring: Measure vital signs, monitor patient progress, and report any changes in condition to the appropriate healthcare team.

Patient & Family Support

  • Health Education: Educate patients and their families on managing conditions, using medical equipment, and maintaining their health.
  • Personal Care: Assist with daily activities such as hygiene, grooming, feeding, and mobility.
  • Emotional Support: Provide comfort, reassurance, and emotional support to patients and their families.
